When you buy adaptive clothing at Mode Ézé Plus, you can benefit from the tax exemption, on presentation of a medical prescription signed by a doctor. This order can be valid for life, without a renewal procedure.

Here’s how :

1. Download and print the form to be completed with the medical prescription.
2. Fill out the form and indicate the name of the beneficiary and the name of the nursing home.
3. Have the form signed by a doctor and write the license number.
4. Send Mode Ézé Plus a scanned copy of the form by email (info@ezeplus.com) by fax (514 303-6536) or bring it when shopping at one of our stores.
